Some files are missing after rescanning...

Hi there,

I have the problem that when i do a rescan the Media Server doesn't add some files to the library...
Most of the files are shown, but still some are missing. I also checked that it is not a particular file type which isn't added, it's just some files. For example one file with .mkv is added and another isn't. Same for .m2ts
Does anyone know why this might happen?

And another question I have is, how do I configure the Media Server (running on Windows) to transcode movies on the fly?
